Our office hours are:

Monday, Tuesday, Thursday 9 a.m. - 5 p.m.

Wednesday 8 a.m. - 3 p.m.

Friday 8 a.m. - 12 p.m.

We see all patients on an appointment basis. Please call our office in advance so that we may reserve a time for you. The office telephone number is (801) 943-5443. We make every effort to be on time for our patients, and ask that you extend the same courtesy.

Earlier and later appointments can be made upon request depending on the availability of Dr. Smith and his staff. The office is closed on weekends and major holidays, as well as during days when the doctor and staff are attending continuing education programs.

If you cannot keep a scheduled appointment, call to notify us immediately. Please give us a minimum of 24 hours notice so that the time may be offered to another patient. There may be a fee charged to your account for same day cancellations and no shows.
