Dr. Mark A. Barnes Dental Program Discount
Welcome to Mark A. Barnes D.D.S.,LTD. Dental Plan. This is a benefit plan, as defined below, not a dental insurance plan: This is an affordable health program alternative for those who do not have access to dental insurance plans. It is a discounted program, which means that there are very few restrictions on the program such as waiting periods and who is eligible. Fees utilized are based upon the fee schedule for the three programs we participate in BlueCrosss/BlueShield, Metlife, and Guardian. For any given procedure the fee utilized to calculate the treatment plan will be the highest of the three schedules. You pay an annual enrollment fee and treatment is provide at the establish program fees. A treatment plan for proposed service will be generated and discussed prior to the actual start of treatment and can change when unforeseen clinical cirumstances require modification from the original diagnosis. Eligibility
To be eligible for coverage, participants must be enrolled in the program and either be a family member and or living in the household. Example Include:
- Legal spouse
- Significant other
- Your unmarried children/dependent who are under the age of 26
Services
The plan offers members a comprehensive package of dental benefits. The plan covers Preventive, Basic Restorative procedures and Major work. All services provided by the practice are included except as described below.
Services not covered
Cosmetic services such as veneers, bleaching and full cast ceramic crowns. Referral and treatment by any specialist is not covered.
Exclusions
The plan is offered to patients who do not have dental insurance coverage. And it is not applicable to any other payment discount such as 12% and 15% if paid at the time of service.
Mark A. Barnes D.D.S.,LTD. reserves the right to discontinue this plan at any time for any member. If membership is discontinued by Dr.Mark A. Barnes, a prorated refund of unused enrollment fees will be provided to the purchaser of the plan.
Enrollment fees
- $225 per single member per enrollment period (Year).
- $450 per two members per enrollment period (Year).
- $800 per family up to 6 members per enrollment period (Year).
Participants are eligible upon payment of enrollment fee for the period noted at the time of enrollment. Eligibility ends when the annual period ends and can be terminated by either party prior to the end of the enrollment period. Eligible renewability is at the option of Mark A. Barnes., LTD. dental plan. Payment if patient portion is expected at the time of serice unless prior arrangements have been made and are appropriate.
